Behavioral Health Home Service
We have made the decision to no longer offer care management services in the form of our Behavioral Health Home program. CFS will no longer offer these services as of May 1, 2023.
We have referred any clients who used this service to other providers in the community and coordinated a smooth transition for them to the provider of their choice. A comprehensive list of all certified Behavioral Health Home providers in Minnesota can be found on the DHS Website.
